Default Ice Maker not working

I have a Whirlpool Gold GD5SHAXNB00 side by side and the ice maker has quit working. When I open the door and hold the flapper in the LED stays on continuously. I set the blades in the 2 o'clock position and unplugged the unit for 3 minutes. After I plugged the unit back in I jumpered the T and H terminals while holding the flapper and light switch in. The blades began to move and water filled the mold. I'm guessing that I need to replace the sensor board(s)...?

Yes, it sounds like your optics are bad.

Here's the cheapest price I found for your optics kit:
Manufacturer's Number: 4389102

4389102

One more issue I noticed. On the left side of the ice maker, where the motor is, the white "gear" mechanism slides over a plastic post with two prongs. One of the prongs is broken off and the "gear" mechanism is not locked in as tight as it probably should be. Is this going to cause a problem. I am fairly confident that the icemaker stopped working before the prong was broken.

Ok, sounds like you have the same issue as the other member I'm helping today.

Look at post#18 here:
[FIXED] KA - KSCS25INSS00 Ice Maker for the mechanically challenged
Does it look like that? If so you will need to order the exact same ice maker module or compete ice maker assembly I posted a link to there.

It does look just like that. Just to summarize, do I need to order both the optic boards and the module/unit?

Yours would just need the optics kit, because the other guys in that thread didn't cycle, and yours did when you jumped T & H.
